Scheduling over the phone
When you call, please have the following information ready:

  1. Patient's name
  2. Reason for appointment
  3. Insurance information
  4. Date of birth
  5. Phone number
  6. Address
Prepare for Your Appointment
You can prepare ahead of time for your appointment. Save time by printing off and filling in our intake forms before your next visit.

Cancellations
If you are unable to keep a scheduled appointment, please let us know as far in advance as possible. This allows us to meet the medical needs of other patients during your cancelled appointment time.

Insurance, Co-payments & Billing
For your convenience, we will file claims for your eye care directly with the insurance carriers. Your insurance will determine whether or not services are covered. You are ultimately responsible for any costs not covered by your health plan.

Co-payments are due at the time of service. Patients who have overdue accounts, are not local residents, or are new patients without insurance will be expected to pay in full at the time of the visit. You will be billed for deductibles and any non-covered services after we have received notification from your health plan.
Please let us know of any changes in your name, address, and phone number, employer, health plan, and group or policy number, so that insurance filing will be more efficient.

For your convenience, we accept Visa, MasterCard, and Discover credit cards, as well as debit cards.

Guarantee
Your complete satisfaction is guaranteed. Your frames and lenses are unconditionally warranted against breakage under normal wear for one year from your purchase date. If you are dissatisfied with your eyewear for any reason, you may exchange it for new eyewear of equal value within 90 days of your purchase.
